Primary Care Initiatives
The Reference Frameworks provide common reference to primary health care professionals to facilitate the provision of continuing, comprehensive and evidence-based care in the community. Through the Reference Frameworks, it is also intended to support family doctors in dissemination of health promotion messages to the public, as well as empowering their patients with chronic diseases and their carers in self-management skills.
The following four Reference Frameworks have been published. Apart from the core documents, there are various modules under which to elaborate on important issues on preventive care and disease management –
- Hong Kong Reference Framework for Diabetes Care in Adults in Primary Care Settings
- Hong Kong Reference Framework for Hypertension Care in Adults in Primary Care Settings
- Hong Kong Reference Framework for Preventive Care for Children in Primary Care Settings
- Hong Kong Reference Framework for Preventive Care for Older Adults in Primary Care Settings
